Cryptosporidium: What You Should Know
Cryptosporidium lives in the intestine of infected humans and animals. It is a microscopic parasite that may be found in soil, food, water, or surfaces that have been contaminated with the feces from an infected host. Cryptosporidiosis is the name of the diarrheal disease caused by Cryptosporidium. Both the disease and the parasite are commonly referred to as "Crypto." An infected person or animal sheds the parasites in their stool. In fact, millions of the parasites can be released in a bowel movement from an infected human or animal. The parasite is protected by an outer shell that allows it to survive outside the body for long periods of time and makes it very resistant to chlorine-based disinfectants. Crypto can be spread: • By putting something in your mouth or accidentally swallowing something that has come into contact with waste infected with Crypto. • By swallowing recreational water contaminated with Crypto. • By swallowing contaminated water or beverages. • By eating uncooked food contaminated with Crypto. • By touching your mouth with contaminated hands. People in buildings that have suffered from a sanitary sewer overflow or due to a flooding event should be aware of this parasite. Child care workers who change diapers and healthcare workers are also at risk. The most common symptom of cryptosporidiosis is watery diarrhea. Other symptoms include: • Stomach cramps or pain • Dehydration • Nausea • Vomiting • Fever • Weight loss Some people with Crypto will have no symptoms at all. While the small intestine is the site most commonly affected, Crypto infections could possibly affect other areas of the digestive tract or the respiratory tract. Public health officials are encouraging consumers to take steps to reduce their risk of listeria infection after recent outbreaks of the foodborne illness. "Listeria is a type of bacteria that can be found in food products, and can cause quite serious foodborne illness in certain populations," says Dr. Nipunie Rajapakse, a Mayo Clinic infectious diseases specialist. "We worry about it especially in pregnant women and their newborns, and people who have weakened immune systems, and in the elderly." She says the most common foods that listeria infection is associated with are usually deli meats, hot dogs, and products that are refrigerated for a prolonged period of time. "Listeria is one of the few bacteria that likes to divide at low temperatures. That’s why we see this being a problem in foods that are refrigerated, and in foods that usually don’t transmit too many infections." CDC Warns About Parasitic Infection Risks In Public Pools
On Thursday, a new report by the CDC warms than people visiting public pools should be aware of a parasitic diarrhea-causing infection linked to pools and water playgrounds which doubled in the US from 2014 to 2016. Michele Hlavsa, chief of the CDC's Healthy Swimming Program, wrote in an email, "Cryptosporidium is a germ that can make people sick with diarrhea for up to three weeks." In 2016, the CDC discovered 32 outbreaks of Cryptosporidium in swimming pools or water playgrounds in the US, compared with just 16 two years earlier. Symptoms include watery diarrhea, stomach cramps, nausea and vomiting, possibly leading to dehydration. Shower with soap before you start swimming. Take a rinse shower before you get back into the water. Take bathroom breaks every 60 minutes. The mixing of chlorine with pee and sweat uses up the chlorine in the pool, which would otherwise quickly kill most germs. Wash your hands after using the toilet or changing diapers (and change diapers in a restroom!).

Diarrhea is a condition in which there is :- Unusual frequency of bowel movements (more than 3 times a day).- Changes in the amount ( more than 200g a day).- Change in consistency( liquid stool).It is a common cause of death in developing countries and the second most common cause of infant deaths worldwide.WHO defines :Having 3 or more loose or liquid stools per day, or as having 3 or more loose stool. Acute diarrhea is an attack of loose motion with sudden onset which usually lasts 3 to 7days but may last up to 10 – 14 days. It is caused by an infection of the large intestine, but may be associated with infection of gastric mucosa and small intestine. The term “acute gastroenteritis” is most frequently used to describe acute diarrhea. Chronic diarrhea is termed when the loose motion is occurring for 3 weeks or more. It is usually related to underlying organic diseases with or without malabsorption. Diarrhea with watery stools and visible blood in the stools is called dysentery. Persistent diarrhea refers to the episodes of acute diarrhea that last for 2 weeks or more and may be due to infective origin. The main causes of this disease are lack of knowledge of hygiene and sanitation, home environment, feeding practices of the parents and under five years of children. It can be caused by chronic ethanol ingestion. Chronic mild diarrhea in infants and toddlers may occur with no obvious cause and with no other ill effects ,this condition is called toddler’s diarrhea. About 1/3 of total hospitalized children are due to diarrheal diseases and 17%of all deaths in indoor pediatric patients are related to this condition. The morbidity rate in terms of diarrhea episodes per year per child under the age of 5 years is about 1.7. Diarrhea is mostly infectious. Pool water making more people sick
WILMINGTON, NC (WWAY) — Pool water is making more people sick across the United States. According to the Center for Disease Control, 32 Cryptosporidium outbreaks were reported in 2016. 16 outbreaks were reported in 2014. The parasite spreads when feces gets into pool water. This often happens when people with diarrhea go swimming. Once infected, a person may experience stomach problems (diarrhea, stomach cramps and vomiting) for weeks. Because Cryptosporidium is hard to kill, the CDC recommends closing infected pools and treating the water with high level of chlorine.

Rotavirus disease is characterized by vomiting and watery diarrhea for 3 to 8 days. Fever and abdominal pain also frequently occur. Additional symptoms include loss of appetite and dehydration. A dehydrated child may also cry with few or no tears and be unusually sleepy or fussy. The incubation period for rotavirus disease is approximately 2 days. The primary mode of transmission is the fecal-oral route. Because the virus is stable in the environment, transmission can occur through ingestion of contaminated water or food and contact with contaminated surfaces or objects. In the United States and other countries with a temperate climate, the disease has a winter seasonal pattern, with annual epidemics occurring from December through June. The highest rates of illness occur among infants and young children, and most children in the United States are infected by 5 years of age. Acute Diarrhea for USMLE
Diarrhea is defined as abnormally liquid or unformed stool that is produced at high frequency (greater than 200g/day) lasting less than 14 days. This handwritten video lecture will review causes, clinical findings, investigations work and management of diarrhea for medical students taking the USMLE ETIOLOGY or CAUSES Infectious is number one cause of diarrhea (90%). Non infectious include medications, toxin ingestion, ischemia. CLINICAL FINDINGS Vomiting and diarrhea is associated with Staph aureus, Bacillus Cereus, Norwalk Virus. Watery Diarrhea is assoicated with Clostridium perfringens, ETEC, and viral diarrhea. Inflammatory diarrhea is caused by campylobacter, salmonella, EHEC, shigella. Inflammatory diarrhea is also associated with pain and fever. Systemic symptoms are reactive arthritis which is associated with slamonella, campylobacter, shigella, yersinia. Reactive arthritis is associated with arthritis, urethritis, conjunctivits. Hemolytic Uremic Syndrome (HUS) is caused by E Coli (0157:H7) and Shigella. Syptoms are related to hemolysis and uremia. EVALUATION of Diarrhea. If mild than no work up is necessary. However, alarm signs require work up. These alarm signs are severe dehydration, bloody stool, Fever greater than 38.5. History of antibiotic use, abdominal pain and immunocompromised patients should get a workup done. First step in evaluation is to take a thorough history to limit and narrow down the pathogen causing diarrhea. If travel history focus on Giardia (Russia, Swimming, Mountanous), ETEC, Norovirus (cruise ship). What type of food have they ate. Meat food poisoning is caused by staph aureus (egg), clostridium perfringens (canned meat), EHEC (hamburgers), Bacillus (Chicken fried rice), Salmonella (eggs and veg), Yersinia (Milk and cheese), Listeria (more comon in pregnant women). Poultry is primarily associated campylobacter. Diarrhea form seafood is associated with vibrio cholera and vibro parahemolyticus. Patients associated with daycare and get diarrhea there are certain pathogens. Shigella, Rotavirus, Norovirus, Giardia, Cryptosporidium. Hospital Acquired Diarrhea is primarily associated with clostridium dificile (anti-biotics). Immunocompromised patients have increased risk of cryptosporidium and isospora. Pathogens associated with fever and diarrhea suggests invasive pathogens such as salmonella, shigella, campylobacter as well as virus and entamoeba histolytica and clostridium dificile. Time can also help determine the cause of diarrhea. If vomiting occurs 1-8 hours after heating then a preformed toxin was ingested. This is most commonly associated with bacillus cereus, S. aureus and Clostridium perfringens. If vomtiing occures 8-72 hours then diarrhea is caused by enterotoxin, primarily Vibro cholera, ETEC. 1-8 days are caused by enteroadherent which prevents absorption of food into gut. This is caused by EPEC and EHEC, Giardia and cryptosporidium. Lab investigation for diarrhea should be focused on details from history. CBC, Hematocrit, and WBC is important. Stool Studies - fecal leukoctyesfor WBC in stool are positive in salmonella shigella but this is also found in Ulcerative colitis and crohn's disease as they all cause some sort of inflammation. If blood is present then dysentary pathogens such as campylobacter, salmonella, enterhemorrhagic E. Coli, and entamoeba histolytica. Stool culture is not done routinely, unless it is very severe infection. Ova and parasites if there is persistent diarrhea that is not going away, recent travel, daycare, homosexual or AIDS patients. If there is bloody diarrhea but leukocyte negative. Fecal antigen test for Giardia, and Rotovirus. Testing for toxin such as clostridium dificile if there is a history of anti-biotics. Fecal osmolar gap and fecal fat is used in chornic diarrhea, not acute. TREATMENT Initial treatment for diarrhea focuses on relieving dehydration with oral rehydration therapy. Use IV if very severe. Gatorade is not optimal, but it is okay. Empirical anti-Biotics can be given for diarrhea, but it is not always required because most are self-limiting. Sometimes anti-biotics can actually worsen symtoms of diarrhea. Most commonly prescribed is flouroquinolones such as ciprofloxacin, norfloxacin. Clostridium dificile of metronidazole, vancomycin. EHEC should not be given anti-biotic due to release of shiga toxin. Culture and sensitivity can guide the rest of treatment.

Discussion on Parasites Hi. I'm Dr. Jared Nielsen here in Heber City in Utah with the Nielsen Clinic. I'd like to visit with you today a very important topic related to parasites. Probably something that most people want to avoid the discussion of because who wants to admit or acknowledge that they've caught something from their pet, or from the water, or from some river, or some form or source that they may not even be aware of. When we look at parasites, we talk about those different organisms that could afflict the GI system. Giardia, amoeba, round worms, different types of flukes, and these organisms try to create within us a lasting relationship. Some references say right now that up to 83, to even 86 percent of Americans have a chronic parasite. According to the World Congress of Tropical Disease and Medicine, at least 45 percent of Americans have chronic amoeba infections and they'll actually carry those to the grave. When we look at afflictions or symptoms especially in children, the inability to rest is a common symptom of parasites. A child will be either unable to fall asleep, particularly, stay asleep or even feel restful. If you're child isn't able to have that good quality of sleep, they're missing out on an opportunity for growth because during the night the body is actually healing. The greatest amount of growth hormone is released and the child isn't able then to recover and mature. If you see your child looking a little ashen, especially unable to sleep, maybe we need to look for parasites. Commonly, we also see skin afflictions. One of the more common skin afflictions that I've seen correlated to parasites is psoriasis. When we ask people about their psoriasis' history, and that's that reddened skin with the flaky appearance to it, we're seeing that especially at the elbows and on the knees. In children, you'll see it often on the scalp and that scalp presentation and down onto the spine, we really look closely as it's a potential indication of parasites. I've been working for quite a while with some different labs regarding parasites. There are several out there that do a good job, but recently, I have been introduced to a doctor, Raphael d'Angelo from Colorado. He's a medical pathologist. His information can be found at Parawellnessresearch.com. We'll give that information again. Dr. d'Angelo had done his tour after finishing medical school in Vietnam. When he was there, he observed several of the military men, the soldiers, coming out of the jungles having these different health afflictions. He started to delve more deeply into the analysis and evaluation of parasitology. What he found he said was just absolutely amazing, the correlation to their wellness and the disease process and these parasites. He is now retired, but has a private lab, the Parawellnessresearch.com. In his personal lab, he actually takes both urine and a stool sample. He prepares those on a slide. He looks at those through the microscope and then stains each of the slides in specific ways to actually identify organisms or different phases of the organism based off of the slide preparation and staining techniques. Great, great technique because the test kits that I've been sending to him now, we've had a 100 percent of the patients coming back with a positive on those. Everything again from worms to different types of amoebas, different types of protozoans, as well as yeasts. He also will send it out for immunoassay to be able to determine if the immune system is reacting, for example, to Giardia or Cryptosporidium. The nice thing about working with Dr. d'Angelo is because he's a medical doctor. He can gives us a recommendation both for the medical prescriptive type of a treatment as well as using a natural compounding pharmacy that uses a natural or a botanical type of a treatment for the patient. Typically, the treatment covers three months. After the three months, Dr. d'Angelo recommends that we do a follow-up parawellness test to prove that we've eliminated the infection and actually then see a resolution of the symptoms.
